Be sure to attach a voided check to the form where indicated.
To recharge your account simply enter the amount you want to recharge
and your registered phone number or PIN.
Optional- To add a Speed-Dial to your account enter the complete number in the form. The Speed-Dial
will be active within 24 hours. To use the Speed-Dial simply dial 1# when prompted for your destination
number. Other numbers can be dialed as normal.
